Hémostase et réponses aux agents antithrombotiques

Thème 2 - Partie 2

L’utilisation des nouveaux antithrombotiques, anticoagulants et antiplaquettaires, concerne un nombre croissant de patients (plusieurs centaines de milliers dans les années à venir). Or il n’existe pas d’antidotes disponibles pour  tous ces médicaments en cas de saignement grave ou dans des situations où un geste invasif urgent est nécessaire. Par ailleurs, le rapport bénéfice / risque qui est bien établi dans les essais cliniques n’est pas nécessairement transposable à tous les patients.

Les travaux développés dans ce thème ont pour objectifs de :

      1. caractériser les antithrombotiques (pharmacodynamie et pharmacocinétique), rechercher des antidotes et développer des tests d’hémostase adaptés (B. Le Bonniec, S. Gandrille)
      2. tester l’efficacité des agents hémostatiques disponibles pour antagoniser l’effet des  antithrombotiques (A. Godier, I. Gouin Thibault, B. Le Bonniec)
      3. comprendre les causes de variabilité de la réponse aux antithrombotiques (I. Gouin Thibault, V. Siguret).

Tout antithrombotique induit un risque hémorragique non négligeable. Les nouveaux antithrombotiques ne sont pas toujours détectés par les tests de coagulation classiquement réalisés. Idéalement il faudrait pourtant pouvoir doser ces antithrombotiques très rapidement dans les salles d’urgence. Ces molécules nécessitent donc le développement de nouvelles méthodes d’exploration

La difficulté de dosage des nouveaux antithrombotiques exacerbe qu’en cas d’urgence thérapeutique aucune antidote n’est encore utilisable en toute sécurité. L’une des thématiques de ce groupe de recherche est précisément la mise au point d’antidotes et l’évaluation d’antagonistes pour ces nouvelles molécules.

Parmi les causes potentielles de variabilité de réponse aux nouveaux anticoagulants oraux figurent les polymorphismes de gènes codant des protéines impliquées dans le transport et le métabolisme de ces médicaments. La p-gp appartient à la famille des transporteurs ABC qui assure  une fonction de pompe d’efflux des xénobiotiques, est un composant clé dans la pharmacocinétique des médicaments dont ces nouveaux anticoagulants en limitant leur absorption intestinale et augmentant leur élimination rénale. Certaines de ces molécules sont par ailleurs métabolisées par les CYP450.

Interface plaquettes et antithrombotiques

Anne Godier, Anne-Céline Martin et Pascale Gaussem s’intéressent à la neutralisation des nouveaux antithrombotiques. Ces derniers incluent des agents antiplaquettaires (prasugrel et ticagrelor) et des anticoagulants oraux directs, inhibiteur de la thrombine (dabigatran etexilate) ou inhibiteurs du facteur Xa (rivaroxaban, apixaban, edoxaban), et sont aujourd’hui très largement  prescrits.

Comme tous les antithrombotiques, ces médicaments, plus efficaces ou plus puissants que ceux qu’ils sont amenés à remplacer, exposent à des complications hémorragiques, parfois graves, dont le traitement est incertain. Leurs travaux sont une réflexion sur la prise en charge du risque hémorragique des antithrombotiques. Ils s’appuient sur des études in vitro et ex vivo, de l’expérimentation animale à travers différents modèles, et sur des études cliniques. Leurs objectifs visent pour partie à évaluer l’efficacité potentielle d’agents hémostatiques déjà commercialisés à neutraliser ces nouvelles molécules, afin de proposer des stratégies thérapeutiques pour la gestion des hémorragies qui leur sont associées.
