Mon équipe se concentre sur les cellules endothéliales, leur origine cellulaire mais également la modulation de la formation vasculaire dans différentes situations pathologiques. Nous étudions notamment, un sous type de cellule progénitrices endothéliales proliférative, pouvant être isolée du sang adulte périphérique ou du sang de cordon, appelés ECFC (endothelial colony forming cell) ou progéniteurs endothéliaux tardifs (late-EPC, à cause de leur propriétés in vitro à être isolée après 7-21 jours de culture). Ces cellules pourraient être utilisées en médecine régénérative.

Projet I : Implication des progéniteurs endothéliaux (ECFC) dans le remodelage vasculaire

Notre objectif est de comprendre comment se construisent les réseaux vasculaires dans les tissus et organes endommagés. Une des situations explorée est la fibrose pulmonaire idiopathique (FPI). Cette maladie interstitielle pulmonaire est associée avec un remodelage constant des vaisseaux. Nous avons pu mettre en évidence en 2013 une modification des éléments du compartiment endothélial circulant chez les patients atteints de FPI. Notre objectif est maintenant d’étudier les propriétés des cellules progénitrices endothéliales dans la FPI ainsi que de comprendre comment la neoangiogenèse peut interagir avec le processus de fibrogenèse. Un des objectifs de ce projet I est également de comprendre comment ces ECFC peuvent interagir avec les cellules périvasculaires et/ou les cellules hématopoïétiques notamment au travers de la voie de l’endogline (projet développé par le Dr Elisa ROSSI).

Projet II: Ontogénie de la lignée endothéliale en situation normale et pathologique

Une de nos objectifs dans ce projet II est de comprendre l’origine cellulaire des cellules endothéliales chez l’adulte. Les seules cellules décrites pour pouvoir former des cellules endothéliales au niveau postnatal sont les cellules souches d’hémangiomes. Nous avons récemment montré  que les VSEL (very small embryonnic like stem cells) isolées de moelle osseuse de patients artéritiques sont capables de se différencier en cellules au phénotype endothélial. Ces 2 types de cellules (HEM-SC et VSEL) ont un phénotype mésenchymateux. D’autres progéniteurs au phénotype mésenchymateux sont capables d’interférer avec l’angiogenèse (Fibrocytes, cellules souches mésenchymateuse) sans se différencier en cellules endothéliales.  L’objectif de ce projet II est donc de comprendre comment des cellules immatures et/ou mésenchymateuses tissulaires ou circulantes se différencient en cellules endothéliales. Un des sous types de cellules issues de tissus pathologiques que nous avons choisis d’étudier sont les cellules interstitielles de valve issues de patients atteints de rétrécissement aortique (Collaboration Pr SUSEN, CHRU Lille, Inserm UMR-S1011).

Projet III: Interface Hémostase, inflammation et cellules circulantes

Nous avons récemment montré que le recrutement de cellules inflammatoires (monocytes et polynucléaires neutrophiles) pouvait être lié au recrutement de cellules vasculogéniques et à la génération de thrombine.

Ainsi, notre objectif dans ce projet III est de comprendre comment des biomarqueurs circulants d’activation plasmatique (coagulation, inflammation) ou cellulaire (Cellules endothéliales circulantes CEC, NETs, amas leuco-plaquettaires…) peuvent être un outil dans la compréhension physiopathologique, le diagnostic et le suivi :

  1. Des pathologies thrombotiques artérielles (ischémie du membre inférieur) et veineuses (Embolie pulmonaire et le suivi des séquelles d’embolie pulmonaire).
  2. De l’hypertension artérielle pulmonaire pédiatrique
  3. De l’hémocompatibilité secondaire mise en place au contact d’une surface bioprothétique. Notre équipe est notamment impliquée dans l’étude clinique de faisabilité du cœur artificiel CARMAT récemment publiée dans le Lancet, qui utilise du péricarde bovin identique à celui utilisé dans les bioprothèses Carpentier-Edwards® (utilisées depuis plus de 20 ans dans le remplacement valvulaire).
