Haemostasis, angiogenesis and vascular differentiation

Theme 1

The group focus on both fundamental and translational approaches to understand vessel pathophysiology in diseases. During the past years, D. Smadja has acquired expertise in the biology of Endothelial Progenitor Cells (EPC) in a fundamental and translational aspect. They are extremely rare and difficult to expand in vitro, in particular in patients with critical limb ischemia [1]. Their ontogeny as well as their implication in the formation of the vascular tree is controversial and not fully elucidated. The identification of the adult vascular stem cell remains open. Our research explores other sources of vasculogenic stem cells in bone marrow or tissues with differentiation potential (i.e. the embryonic-like stem cells VSEL) and the impact of the haemostasis pathway on the vascular compartment, to solve remaining therapeutic challenges such as the haemocompatibility of bioprosthetic materials and the treatment of ischemic diseases.

The mains projects are organised as follows: 

    1. to identify new interactions of haemostasis with the angiogenic process,
    2. to analyse the functional vasculogenic capacities of Very small embryonic-like stem cells (VSELs),
    3. to evaluate the role of vasculogenic cells in the haemocompatibility of bioprosthetic valves and mechanical support,
    4. to understand the pathophysiology of arterial and venous thrombosis, exploring among other on the identification of the actors involved in arterial aging.

Part 1: New interactions of haemostasis with the angiogenic process:

Thrombus formation and wound healing are characterised by bidirectional interrelations between platelet / coagulation activation and angiogenesis. This project aims at studying the importance of haemostasis in its interaction with the angiogenic process and EPC differentiation. We have studied the involvement of Endoglin present on endothelial cells in normal and pathological conditions, in the intercellular adhesion of endothelial cells / EPC to mural cells and mesenchymal stem cells in revascularization. We now proposes to evaluate the importance of physical interaction of endoglin with cell membrane and explore the role of both membrane and soluble endoglin in the vasculogenesis process as well as in the haemostasis and arterial thrombosis.

Part 2: Very small embryonic-like stem cells (VSEL) as a source of vascular cells:

Because of their capacity to differentiate into endothelial cells and to promote post-ischemic revascularisation in a murine model of hindlimb ischemia [4], VSEL could represent a clinically relevant alternative to embryonic stem cells and induced pluripotent stem cells for cell therapy. Recently a group confirmed the endothelial differentiation of MSC derived from VSEL [5]. There are two major hurdles before these cells can be widely used in the clinic. First, they are very rare, and second, they reside in adult tissues in a quiescent state, due to erasure of methylation in regulatory sequences of parentally imprinted genes, a mechanism well known to maintain the quiescent state in primordial germ cells. Ratajczak’s group showed that it is now possible to expand VSEL in vitro when exposed to valproic acid and pituitary hormones [6]. We thus aim to explore their vasculogenic capacities and to investigate the possibility to use them as biomarkers of tissue regeneration.

Part 3 Valvular interstitial cells (VIC) and haemocompatibility of biomaterials:

The implantation of artificial valves or biomaterials can result in several complications including thrombosis.
The native aortic valve displays a layered architectural pattern of cells, including valvular endothelial cells (VEC) at the blood contacting surface as well as deep VIC. VIC participate to calcification process, have the ability to support EPC angiogenic properties and could be at the origin of both endothelial and perivascular lineage.

Aortic Valve Stenosis (AVS) can be treated by valve replacement using bioprosthetic material made with pericardial tissue from bovine origin. Such valves induce an acquired haemocompatibility and allow a lower dose of anticoagulant treatment to be given in patients, in contrast to mechanical valve [7]. However, after several years, these valves can become dysfunctional showing the same phenotype than the native one from AVS: fibrosis, neoangiogenic process, etc, recapitulating the classic history of native valve degradation but on a bioprosthetic material.

Therefore, our project aims:

  1. to better define the role of VIC in the occurrence of this dysfunction as well as to determine their ability to support angiogenesis in aging bioprosthetic valves,
  2. to study the haemocompatibility and complete endothelialisation of the cardiac bioprosthesis Carmat-total artificial heart and of extracorporeal CO2 purification technique.

Part 4: Pathophysiology of arterial and venous thrombosis:

Arterial thrombosis: Identification of the actors involved in arterial aging is a first essential stage in therapeutic approach. Firstly, by creating murine models of aging with a murine model characterized by the endothelial deletion of TXNIP (Thioredoxin-interacting protein), we were able to show that the Thioredoxin/TXNIP system was a major player in aging by protecting the artery from oxidative stress, inflammation and loss of aortic compliance induced by arterial aging. Moreover, using a model of ischemia of the lower limb, we observed that the endothelial deletion of TXNIP protected from the defect of post-ischemic revascularization during aging, confirming that endothelial TXNIP was a therapeutic target [8]. Our main project is to characterise the specific involvement of endothelial TXNIP and aging in our EC-KO TXNIP mice, to establish its involvement in angiogenic repair in vivo and ex vivo as well as to identify the actors involved with TRX-TXNIP system in this protection. We are also investigating whether the inhibition of TXNIP could be a strategy for the treatment of neuronal and vascular lesions after stroke.

Chronic venous thrombosis: O. Sanchez, B. Planquette and D. Smadja are partners of INNO-VTE that is a new F-CRIN-labelled network in France, aiming to develop translational clinical research and european-wide studies on venous thrombosis. Our expertise in chronic venous thrombo-embolism (VTE) led us to investigate the link between pulmonary vascular disease and the observed perfusion sequelae retained dyspnea frequently observed in patients with PE (Dyspnée 2 Study, O. Sanchez, clinical research initiation contract (CIRC)). We think that endothelial dysfunction is an important clinical and pathophysiological partner of sequelae in Pulmonary Embolism. Embolic sequelae can lead to Pulmonary Arterial Hypertension.
